Q. Can you please provide a clearer definition of a brochure. Is there a page limit/size restriction on a brochure? |

A. A brochure is a pre-printed pamphlet or booklet containing descriptive or advertising material for a company. There is no page limit/size restriction, but it must be attached as an appendices to the RFP #1and must fit within the container chosen by the Design-Build team for this submittal. |

A. If teaming agreements or contracts exist, they should be included. These documents are excluded from the page limitations. |

A. MSHA will accept the EMFfrom either the Workman's Compensation insurance carrier or it's agent. |

Q. From Pre-Proposal Meeting's, Top of page 9 (c.); What format is required to provide records of Erosion and Sediment Control Quality Assurance ratings. Does it need to be provided on the actual Quality Assurance Rating forms or can we present it in either a tabular or chart format? |

A. Presenting past performance records of Erosion and Sediment Control Quality Assurance ratings is left up to your discretion. MSHA does not require the actual Quality Assurance Rating forms for each project. Providing this information in tabular or chart format is acceptable; however, you are required to relate your past performance with it's associated MSHA contract number and project description. |

Q. Regarding Item 9 in SF 255, should we only list MSHA work? What do we do if it does not fit on one page (either at 12 point font of smaller)? |

A. Item 9 in SF 255 should only list work performed for MSHA. In order to avoid exceeding one page, limit your work performed for MSHA to relevant work. |

Q. The proposal requests 12 point font size. Is this applicable to the SF 254, SF 255, and Organization Chart? |

A. MSHA will allow a minimum font size of 9 point only for the SF 254, SF 255, and the Organization Chart. All other proposal requirements will need to meet the minimum 12 point font size as specified on page 13 of the RFP Step #1. |

Q. On page 7 under Experience and qualifications documentation items (i) and (ii) indicated we must provide Standard Forms 254 and 255, which are currently obsolete. GSA has replaced both these forms with SF330. |

A. The Maryland State Highway Administration continues to accept the Standard Forms 254 and 255. For this procurement, we will not accept SF330. |
